Your protected health information (PHI) includes any demographic data that could identify you and is related to your past, current or future physical or mental health; healthcare services you’ve received; or any past, current or future payments for healthcare services rendered to you.

- You have the right to view or request a physical or electronic copy of your medical records and other health-related information we possess. We’ll guide you on how to do so.
- We will provide a summary or a copy of your health records, generally within 30 days of your request. A reasonable, cost-based fee may apply.
- Ask for corrections in your medical records.
- You can request us to amend any inaccurate or incomplete health information. We’ll guide you on how to do so.
- We may deny your request. Should that happen, we will provide you with a written explanation within 60 days.
- Request confidential communications.
- You can specify the method (like home or office phone) or location where we should send your communications.
- We will comply with all reasonable requests.
- Ask us to restrict the use or sharing of your data.
- You can request us to avoid using or sharing certain health information for treatment, payment, or our operations.
- We are not obliged to agree to your request, and we may decline if it would affect your care.
- If you pay in full for a service or health care item, you can request us not to share that information for payment or our operations with your health insurer.
- We will comply unless a law obliges us to share that information.
- Request a list of instances where we’ve shared your information.
- You can request a list detailing the instances where we’ve shared your health information over the past six years, the recipients, and the reasons for sharing.
- Our list will exclude disclosures made for payment, treatment, health care operations, and certain other types of disclosures (like ones you requested). We’ll provide one such list for free every year. Additional lists within 12 months will incur a reasonable, cost-based fee.
- Request a copy of this privacy notice.
- You can request a physical copy of this notice at any time, even if you’ve opted for an electronic copy. We’ll promptly provide you with a physical copy.
- Appoint someone to act on your behalf.
- If you’ve given someone medical power of attorney or if someone is your legal guardian, they can exercise your rights and make decisions regarding your health information.
- We will verify the person’s authority to act on your behalf before taking any action.
- File a complaint if you believe your rights have been violated.
- If you believe we’ve violated your rights, you can lodge a complaint with us using the contact information mentioned on the first page.

- You will not face any retaliation for filing a complaint.
Your Options
For specific protected health information, you can dictate your preferences about what we disclose. If you have a distinct preference for how we handle your information in the circumstances mentioned below, please inform us.
In these instances, you have both the right and choice to instruct us to:
- Disclose information to your family members, close friends, or others involved in your care.
- Disclose information in a disaster relief scenario.
- Include your information in a hospital directory.
- Contact you for fundraising initiatives.
If you are unable to communicate your preference, we may still disclose your information if we believe it to be in your best interest. We may also disclose your information when necessary to mitigate a severe and imminent threat to health or safety.

In terms of fundraising:
We may contact you for fundraising initiatives, but you can request us not to contact you again.

- We can use and share your health data to operate our practice, enhance your care, and communicate with you when necessary.
- Invoice for your services
- We can use and share your health data to invoice and receive payment from health plans or other entities.
- We are permitted or obliged to share your information in other ways – typically in ways that contribute to the public good, like public health and research, subject to strict legal conditions and laws:
- Assist with public health and safety issues.

- Assist with product recalls.
